Understanding sesamoid injuries

The sesamoids are small bones beneath the big-toe joint that bear substantial pressure during push-off. Pain may develop gradually or after an acute injury.

What to expect during evaluation

Focused examination and specialized X-ray views assess the sesamoids. MRI, CT, or comparison imaging may distinguish stress injury, fracture, and normal variants.

Treatment options

Treatment depends on the diagnosis, severity, activity goals, overall health, and prior care. Options may include:

  • Activity modification
  • Offloading pads or orthotics
  • Protective footwear or boot
  • Medication when appropriate
  • Surgery for selected nonhealing or arthritic cases
