Understanding plantar fasciitis
Plantar fasciitis is a common source of heel pain caused by irritation where the plantar fascia supports the arch and attaches to the heel. Symptoms are often most noticeable with the first steps after rest, but other problems can mimic it, so treatment begins with confirming the diagnosis. A heel spur may appear on an X-ray but is not usually the source of plantar fasciitis pain.
Common signs and symptoms
- First-step pain in the morning
- Tenderness beneath the heel
- Pain after prolonged standing
- Symptoms that increase after activity
What to expect during evaluation
Evaluation includes the location and pattern of pain, ankle flexibility, arch mechanics, gait, footwear, and areas of tenderness. Weight-bearing X-rays may be used when another cause of heel pain is suspected.
Treatment options
Treatment depends on the diagnosis, severity, activity goals, overall health, and prior care. Options may include:
- Activity and footwear modification
- Plantar fascia and calf stretching
- Supportive inserts or custom orthotics
- Medication or injection when appropriate
- Surgery for carefully selected persistent cases
